Kindergarten Immunization Requirements

An up-to-date immunization record that includes all required vaccines for school attendance or a signed exemption form is mandated for school entry by Ohio Law. 

Please submit shot records at registration.

PLEASE NOTE: Your student may require additional vaccines for school entry. Please contact your student’s medical provider to make sure your student is up-to-date before the first day of kindergarten.

Vaccine types and typical number of doses required for school entry following the recommended CDC schedule:

  • DTaP (Diphtheria, Tetanus, Pertussis)
    • 5 doses 
  • Polio (IPV)
    • 3 or more doses of IPV (typically 4 doses given)
  • MMR (Measles, Mumps, Rubella)
    • 2 doses
  • Hepatitis B (Hep B)
    • 3 doses
  • Varicella (Chickenpox) or documented history of disease
    • 2 doses

Be sure to take your student’s immunization record, parent/guardian photo ID, and insurance card with you to the clinic visit.
